Problem - Solution. These are the proper first and last steps in a problem-solving scenario. Too frequently alternative solutions get mixed up in problem definition. I have attended meetings where the issue became so confused that at the end of the meeting everyone simply got up and left. I'm not really sure if they thought they solved the problem or not. Obviously the only problem that is solved is the problem that is defined properly and clearly to all parties involved. It takes a great deal of discipline to facilitate good problem definition. Because our minds work so quickly it's inevitable that people will jump ahead. Sometimes it's even necessary for clarity. Just be sure to bring the group back to the definition prior to finalizing a solution.
