Does your team own the plan?

Poll your change team. Do they fully understand the tasks assigned to them? If not I’ll bet they had little to do with creating the tasks in the plan. Sometimes that’s not always possible but can you get the same effect by having project team members explain their upcoming tasks in small groups? This sharing brings clarity and unmasks misunderstanding while there is time to recover.
